I've had some problems the other way around - fat boy
or paper case covers that I needed converted to slim
jewel case size.

What I did was modify existing covers using Microsoft
Photoddraw.  Many of the new issues, say from EV,
aren't for jewel cases.  What I do is cut off the one
side tab, resize the front so that it fits within a
12.07cm X 12.07cm square, and fill in the background
with a prodominant colour from the front cover image,
so that it becomes consistent.

Then I resize the back, resize the side tab that I cut
off, make a copy of the tab so that I have two in
total, and assemble the back cover, than save.

It all works quite well, all things considered.  I
imagine if you want to go from slimline jewel case to
fat boy, you could use much the same principles, but
kind of in reverse.
