CD Damage Question

Hi All, I have a question. Has anybody encountered weird scratches,
very small
but damaging to the top of their cd's. This has happened to a couple
of my CD-
R's and normal commercial Cd's. I keep them in separate Case Logic
cd books.
I thought I took special care in taking good care of them. Does
anyone know
how I can prevent this from happening? Thanks!
